New Patients

When you are ready for your first appointment, to insure that you are seen promptly we ask that you come in 10 minutes early to complete any intake forms.

 

Additionally, you may print out the below New Patient Form and bring it into the office already filled out and we can add it into your private patient chart.

Out of network patients:

IMPCS  can continue to be your medical care specialists if your insurance plan accepts “out-of-network” providers.

 

Most plans have coverage for patients who see doctors outside of their network.

 

Patients who have out-of-network benefits may have to pay deductibles and co-pays for services by a non-participating provider.

 

Please consult with your plan provider about their out-of-network benefits. If you have questions regarding the benefits of your plan, we have a knowledgeable billing department that can assist you to be able to help you.
